"hard to reach" children and their families

What successful activities is your state or community using to find and identify eligible children who are part of “hard to reach” families – e.g., those who are homeless or highly mobile, living in extreme poverty, immigrants with limited English proficiency, teen parents, etc?

Two strategies that can be effective are the following:
1. Routinely collect a second point of contact for the family such as a neighbor or friend so that when the family moves you have a way to find out current information.
2. When calling by phone and there is no answer, hang up without leaving a message and call right back. Many people will answer the second time.
